I think one of the biggest differences between Japan and the U.S. is the greater visibility of handicapped and disabled people on the streets. It's not uncommon to see someone visually impaired walking by themselves along the sidewalk, or someone else standing in front of a vending machine and carrying on a losing argument with it.
A couple days ago, I was walking through Tenmonkan, wearing one of my Cog is Dead t-shirts. Some younger guy walking towards me suddenly started pointing at my chest. From what I could tell, he was acting like he was looking in a mirror, and had to point back. I didn't stop to ask him if my guess was right or not.
